Online Registration for Youth Retreats

You can register for our upcoming retreat in a few easy steps:

Open and save the Registration Form on your computer:                                 
1. To properly save the form on your computer, right click on the link below and select “Save Target As.”  Then, save the form on your desktop. Open the saved form in Adobe Reader and fill out the Registration form .

2. When you get to the signature box, the parent and student must both digitally sign the form. To do this, click in the parent signature space. A sign document box will appear with your name. If the proper name does not appear, click the down arrow and select New ID. Select the option to Create a new digital ID. Once completed you will be able to digitally sign the registration form. You will need to repeat the process for the student signature. Please note: The parent and the student must personally sign the box. Do not sign it for someone else, as the signature acknowledges that the parent and student have read and understand our retreat policies.
3. Once completed click to save the form.  Next attach the completed form in an email addressed to Registration@faithfellowshipweb.com.

Make payment for the retreat:
1. You are able to either make a deposit, pay the retreat fee, or pay the outstanding retreat balance. All you need to do is click on the appropriate link and follow the instructions. In the description box please put the name of the student(s) that you are paying for so it can be matched with the proper registration form.
